We are looking for an experienced Principal .NET Full Stack Engineer to lead the architecture, development, and ongoing improvement of this platform. This is a hands-on technical leadership role for an engineer with deep expertise in the Microsoft technology stack, Azure cloud architecture, Windows-based applications, and secure healthcare software.
You will work directly with a U.S.-based engineering team to build reliable, scalable, and secure systems that support clinical workflows.
What You'll Do
Lead the architecture, development, and maintenance of full-stack applications using C#, .NET, ASP.NET, and Microsoft Azure.
Design and maintain secure, scalable, multi-tenant cloud applications, REST APIs, and SQL-based data solutions.
Develop and support Windows Services, background agents, and desktop application integrations.
Build and maintain reliable data synchronization between Windows-based diagnostic instruments and Azure-hosted systems.
Implement authentication, authorization, role-based access control (RBAC), tenant isolation, and protection of sensitive health information (PHI).
Develop PowerShell scripts for deployment, automation, troubleshooting, and infrastructure management.
Diagnose and resolve production issues across cloud infrastructure, backend services, databases, and Windows environments.
Improve application security, automated testing, CI/CD pipelines, and software release processes.
Conduct code reviews, establish engineering best practices, and mentor other developers.
Collaborate with U.S.-based engineering and clinical teams to deliver dependable healthcare software.
What We're Looking For
10+ years of professional software development experience, including technical leadership or architecture ownership.
Strong hands-on expertise in C#, .NET, ASP.NET, REST APIs, and SQL.
Proven experience building and maintaining applications on Microsoft Azure, including multi-tenant cloud architectures.
Practical experience developing or supporting Windows Services, desktop applications, or Windows-based background agents.
Strong PowerShell scripting and automation skills.
Experience implementing secure data synchronization, certificate-based authentication, and troubleshooting Windows environments.
Solid understanding of application security, RBAC, tenant isolation, and protection of sensitive data.
Experience with GitHub, GitHub Actions, CI/CD, and Jira.
Healthcare technology experience involving digital health, medical devices, clinical software, EHR integrations, HIPAA, or PHI-sensitive systems.
Strong problem-solving skills, architectural judgment, and the ability to lead technical initiatives hands-on.
Fluent English (C1 or equivalent) and the ability to collaborate effectively with U.S.-based teams.
Nice to Have
Experience with medical devices, diagnostic instruments, or clinical decision-support platforms.
Knowledge of HL7, FHIR, and EHR integrations.
Experience with data replication, offline synchronization, or device-connected applications.
Familiarity with X.509 certificates, OWASP ZAP, Microsoft Defender for Cloud, and security remediation.
Experience with Selenium, Azure DevOps, or regulated software development and release processes.
What We Offer
100% remote work, with candidates based in Latin America preferred.
Full-time, long-term B2B contract.
Direct collaboration with a U.S.-based engineering team.
An opportunity to lead the development of healthcare technology used in clinical environments.
10 paid U.S. public holidays.
Vacation and sick leave are unpaid.
Security and Compliance Requirements
Due to the sensitive nature of the platform and its healthcare environment, the engagement includes the following requirements:
Use of an SRS-provided computer with required Microsoft security settings.
Criminal background check before engagement.
Completion of annual HITRUST security training.
Virtual home-office security assessment, including screenshots of home-network settings.
Work Arrangement
Location: Remote Latin America preferred.
Employment type: Full-time, long-term B2B contract.
Collaboration: U.S.-based engineering and clinical teams.
Language: Fluent English (C1 or equivalent).
If you have extensive experience with .NET and Azure, understand the complexities of Windows-based systems, and want to apply your technical leadership to healthcare technology, we'd like to hear from you.
